The Price List, Remittance Form and the ordering form for OKU Membership Certificates & Insignia are available on line at the links below. To help calculate orders, refer to the remittance form, which should be filled out by the chapter secretary.  A copy should be mailed with the order along with payment. A copy should be retained for your component chapter files.  Please consult this schedule and calculate carefully when sending remittance with your orders.

On the Membership Certificates & Insignia order form, names should be grouped and identified according to a membership category (e.g., student, faculty, honorary, life) and each group should be alphabetized.

A graduation date column refers to the date the person has or will be graduated from dental school and is important for our records.  The election date will appear on the key (if engraved) and certificates.  To speed delivery on your keys, you may wish to order them unengraved and have them engraved locally.  (Please remember that you still must send the order form with the names of members so they can be recorded in the Supreme Chapter records.)  If you elect to engrave the keys locally,  you may deduct $2.00 per key from the prices quoted. Record the person's first name, middle initial, and last name (no degrees).  (This is very important for the Supreme records as we have many identical names).  Check spelling carefully.  You will be charged for correction if the error is yours.  Lilac ribbons for the keys are available at $2.50 each.

If you wish to order lapel pins, indicate the number you need on the order form (they are not engraved).  The Secretary-Treasurer of the Component Chapter is responsible for making certain that pins are furnished to members only.  (We must also have names and election dates for the pins for Supreme Chapter records).

Orders are accepted only upon official order forms (printed from the website).  Your order will be forwarded the same day it is received, but the factory requires FOUR WEEKS just to process the order.  Mailing time is additional.    Therefore, please submit your order promptly  after elections so that you will receive the items for your presentation ceremonies in a timely manner. Please use the street address of your school or home, as UPS will not deliver to post office boxes.   

OKU histories may be ordered at $.50 each.
