INTERACTIVE FORM

 

In order to use the interactive feature of this form:

1.  When form opens, select Text Tool icon,

2.  Use "TAB" key to activate cursor.  Cursor will appear in first text box,

3.  Use "TAB" key to move cursor from text box to text box,

4.  To move cursor backwards, Press (Shift)TAB,

5.  Font is auto select.  It will fit to the space available,

6.  Print form when complete,

7.  Mail completed form with payment to:
